Requesting Medical Records
Individuals who have been a patient at Big Sky Surgery Center have a right to
access or receive copies of their medical records except under certain very
limited conditions. A written HIPAA compliant release form is required.
You may obtain a request form by stopping by our front desk M-F 6am-5:30pm.

There is no fee associated with receiving copies of your medical record, unless more than 1 copy is requested,
then the charge is as follows: 50¢/page + $5-15 handling fee. This information will be provided to you within 10
days of receipt of a HIPAA compliant record release form.
